Form
Using the Executable File
This file is approximately 500K. It was created in OmniForm. Use this
application form if you are using Windows NT, Windows 2000, Windows ME,
Windows 95 or Windows 98. These are program files and you must open them
from your desktop. Do not attempt to open them with Word or any
other word processing application.
As the fonts used in the forms are Times New Roman - 12-point type, you
may want to use this font for consistency.
You must print the application, sign it, and submit it to us. We must
have original signatures on the application form submitted to us. We cannot
accept applications submitted to us on disk or via e-mail.
For programs with deadlines, signed applications accompanied by all appropriate
support materials must be in the OAC office no later than 5 p.m. on the
deadline date. When a deadline falls on a weekend or holiday, the OAC
will accept applications until 5 p.m. on the next business day.

- Downloading the file:
a. When you select and click on the desired application form, you will
be asked if you would like to open the file or save it to your computer.
You may wish to download the file into a folder on your C: drive or
simply open it without saving it." At the prompt: What would
like to do with this file? Select Open or Save this file
to disk. Click <OK>.

b. You will get the File Download dialogue box. You do not need
to insert a disk, store the file on your hard disk where you can find
it. You will get the Save As dialogue box.
c. To complete the Save As dialogue box, when asked we suggest you create
a folder in C:\Temp named "FY#." (where # represents whatever
fiscal year you are applying in). This is where you will put your OAC
forms for the appropriate application year.
d. Click on the downloaded FormC.exe, FormD.exe, Contsumm.exe or OhioHeritageFellowships.exe
file (or the appropriate final report file name) in your C:\Temp directory.
- Opening the file:
a. Go to the FY2006 folder and click on the file to open it. As these
are program files, you must open them from your desktop. You will
not be able to open them with Word or any other word processing application.
b. On the OmniForm Mailable Filler opening screen, click on <OK>.
This will open the form.
- Navigating the Form
- Fill out the application by checking boxes with your mouse or
filling in a blank. The areas to be completed are highlighted in
yellow.
- To move to a particular page of the form, click on the page numbers
at the bottom of the form. The selected page is highlighted in black.
- To select a check box, place your mouse pointer over the box to
be checked. The pointer will change to a hand. Clicking on the box
places an "X" in the box. If it is a multiple choice questions,
you can only select one answer.
- To add information, tab or move your mouse to the field and click
into the area. You can then enter your information
- Copying and Pasting
- You can compose any narrative sections on a word processing program
(like Microsoft Word) and copy and paste them into these sections.
- Make sure that you fit the text into the spaces provided.
- The text will be automatically converted to Times New Roman
12-point type.
- Saving Your Work
- To save your work, use the File:Save As command.
- Save your work under a new file name, such as "FY2006HeritageFellowship"
(the fiscal year and the program area for which you are applying).
- Use a different file name for each application if you are submitting
multiple applications to the OAC.
- Printing the Application
- Once you have saved all of your changes, print your application
- Use File:Print from the menu or click the Printer
icon to print the form.
- Proof your work!
- Proofread the application after your print it! Make any corrections,
save the form and print it again.
- Remember to check the instructions regarding signing and submitting
the form.

The Columbus Metropolitan Library, Ohio Arts Council and Friends of the Library have once again partnered to bring arts opportunities to artists and the public by presenting a juried art show at the Columbus Metropolitan Library Main Library. In 1998 the two organizations collaborated to create the Ohio Online Visual Artist Registry (OOVAR), a database that showcases the work of more than 700 artists from Ohio and around the world.
